Privacy Policy

All healthcare providers are required by law to maintain the privacy of your medical information. Your provider at NWPWC will hold your medical information in strict confidentiality and will only exchange that information with other providers or health care team members with your written permission.

It is critical for you to know your rights. By complying with the Health Insurance Portability and Accountability Act (HIPAA), your provider will ensure the following:

  • The right to access your records – Patients have the right to view and obtain copies of their own records.
  • The right to request restrictions- Patients can put restrictions on who has access to their records
  • The right to confidential communication – Patients have the right to receive communication about their records in a confidential manner.
  • The right to amend the record – Patients can request amendments to their records when they disagree with the content, but at the same time, medical professionals have the right to deny those requests. If your request is denied, we will provide you with a written explanation. You may respond with a statement of disagreement, which will be added to the information you want changed.
  • The right to an accounting of disclosures – Patients have the right to know everyone that our office disclosed information to.

Your provider may reserve the right to change specific privacy practices at any time and will keep you informed if changes occur.
